Background: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) reduce major adverse cardiovascular events (MACE) in type 2 diabetes (T2D); whether semaglutide confers cardiovascular benefit in patients with established atherosclerotic cardiovascular disease (ASCVD) and overweight or obesity in the absence of diabetes has not been established. Results: Over a mean follow-up of 3.3 years, the primary endpoint occurred in 6.5% of the semaglutide group and 8.0% of the placebo group (hazard ratio [HR] 0.80; 95% confidence interval [CI] 0.72–0.90; P<0.001). Mean change in body weight was −9.4% versus −0.9%. Adverse events leading to permanent discontinuation occurred in 16.6% versus 8.2%, predominantly gastrointestinal. Conclusions: In patients with pre-existing cardiovascular disease and overweight or obesity without diabetes, weekly semaglutide 2.4 mg was superior to placebo in reducing the incidence of death from cardiovascular causes, non-fatal MI or non-fatal stroke.
